REGISTERING WITH THE PRACTICE
If you normally reside in our catchment area , you could apply to join our list. If you are a citizen of a foreign country you may still be entitled to register as an NHS patient.
Full registration
If you have your medical card bring this and those of any other in your residence who wish to register, to our reception during Surgery hours.
- If you have misplaced your medical card you will have to fill out an application form, one for each applicant.
- You will be required to provide your post code, and the name and address of your previous G.P.
- You then will be offered an appointment to see our practice nurse for a routine medical check.
- This is a part of our regular procedure before seeing a doctor.
Temporary registration
If you are a visitor to the area you could register as a temporary patient up to a period of three months. you could also obtain Contraceptive or Maternity services as a temporary patient.
To register, speak to one of our receptionists. You will be required to provide your temporary and your home/permanent address and the name and address of your G.P.
With all above cases, it may not be possible to accept every applicant to join our list, in which case you will be notified promptly.

Our practice runs an appointment system, therefore you will need an appointment to see a Doctor, Practice Nurse.

Urgent appointments
If you think you need to see a doctor the same day for an urgent medical condition, please speak to one of our receptionists, over the phone or in person, and we will try and fit you in.
Please use this service only for emergency medical conditions that cannot wait until the next available appointment.
OBTAINING A REPEAT PRESCRIPTION
Since our repeat prescription system is computerised rather than automated, you will need to request prescription 48 hours ahead of the due date.
Please try to bring in your repeat prescription print out, which has a tear out section on the right hand side. Tick the items you need and post it in the box at the reception area of the surgery.
If you are not sure of the drug names please bring along the empty container/box from your chemist and and speak to one of our receptionists.

>IF YOU WERE RECENTLY DISCHARGED FROM A HOSPITAL OR YOU ATTENDED A CLINIC AND YOUR MEDICATIONS WERE CHANGED PLEASE INFORM OUR RECEPTIONIST AS SOON AS POSSIBLE. IF THE HOSPITAL GAVE YOU A NOTE OR A NEW LIST OF DRUGS, PLEASE HAND THEM OVER TO OUR RECEPTIONIST PROMPTLY .
IN THE INTERESTS OF SAFE PRESCRIBING REQUESTS WILL NOT BE TAKEN OVER THE TELEPHONE.
OUT OF HOURS SERVICE
General Practitioners in the UK provide a 24 hrs medical service. During weekends/night time when your own GP is not available the needs of the practice population is delegated to TAKE CARE NOW (TCN)
The out of hours needs of our practice is covered by " TAKE CARE NOW (TCN)" which is a well established firm and employs well trained and experienced nurses and GP's after 6 PM to 8 AM Monday to Friday. And after 6 PM Friday to 8 AM Monday, including bank holidays. All other times contact us at the below .
If you should need any medical help during out of hours please ring our surgery number 0844 477 3035 and you will be advised on how to contact the out of hours services. From here you can ask for advice from a nurse or a Doctor, or you can arrange to see a doctor at a primary care centre, or request a home visit.

REQUEST A HOME VISIT
Home environment is not always the best place to conduct a consultation, examination or to arrange appropriate tests. It is therefore advisable to limit requests for home visits to those seriously ill house bound patients who cannot possibly attend surgery.
ARRANGING A HOME VISIT BY TELEPHONE OR IN PERSON
If you think a home visit is necessary please inform the surgery before 10.30am. This will give adequate time for the doctors to plan their visits. Naturally, it will be the doctors' decision over need and priorities, so it would be very helpful to let know the reason for the home visit.
Please ensure you give the receptionist the exact address of the patient and a contact telephone number.
